Scope of Services
Peak Family Transport LLC provides scheduled, prearranged, non-emergency medical transportation (NEMT) for passengers who are medically stable and appropriate for transportation in a standard passenger vehicle or wheelchair-accessible vehicle, when available and requested.
Our services are designed to assist passengers traveling to and from medically necessary healthcare-related destinations, including but not limited to:
  • Primary care appointments
  • Specialty physician appointments
  • Hospitals
  • Outpatient surgery centers
  • Dialysis centers
  • Physical, occupational, and speech therapy
  • Imaging centers
  • Laboratory services
  • Behavioral health appointments
  • Dental and vision appointments
  • Rehabilitation facilities
  • Skilled nursing facilities
  • Pharmacies
  • Other medically related appointments
Peak Family Transport LLC also provides transportation authorized by participating commercial insurance plans, Medicare Advantage plans, workers’ compensation carriers, healthcare organizations, hospitals, and contracted transportation brokers, subject to applicable authorization requirements.
Peak Family Transport LLC is not accepting new Colorado Medicaid broker transportation at this time.
Peak Family Transport LLC does not provide emergency medical transportation, ambulance services, stretcher transportation, wheelchair transportation requiring specialized medical staffing, transportation requiring continuous medical monitoring, or transportation for passengers whose medical condition requires emergency medical intervention.
Passengers must be medically stable and able to safely travel using the level of transportation reserved. If a passenger’s medical condition changes such that emergency medical services are required, Peak Family Transport LLC reserves the right to decline or discontinue transportation and immediately contact 911 or other emergency responders when necessary to protect the health and safety of the passenger, driver, and public.
Transportation services provided by Peak Family Transport LLC are intended solely for non-emergency situations and should never be used as a substitute for emergency medical care or ambulance transportation.
